Chronic Disease Management

Our Multi-Step Approach:

  1. Determines the requirements for a disease management program, including goals, data collection and management, treatments that will be included, and the level at which this CDM will be managed.
  2. Identifies key measures for each condition including individual tests, schedules, and actions to take when test results are returned out of normal bounds.
  3. Works with key personnel at every level of the organization including physicians, mid-level practitioners, nurses, and allied health professionals.
  4. Implements technology to support the system, HIE, including Electronic Medical Records, access portals to information, and direct input by medical devices.
  5. The distinction of Rural Health IT is the means by which we obtain funding for our clients and continue ongoing support for the program. This involves coordination and communication among:
